Recurring Charge Terms and Conditions

• VEOS® License offered at $3,275.00 for the first 3 years. After 3-years, you will be charged annually at $800.00 USD /year
• I hereby authorize Canfield Scientific, Inc. to charge the indicated card for the VEOS® License including any applicable taxes.
• I agree that this is a periodic charge that will be made according to my billing cycle, and in order to terminate the recurring billing process, I must either cancel my agreement or arrange for an alternative method of payment.
• I agree to the terms and conditions set forth in the Canfield Scientific terms of service, and I understand that I will forfeit any fees paid to date if my agreement is terminated as a result of a terms of service violation.
• I understand that Canfield Scientific, Inc. will not mail me any invoices or bills.
• I authorize Canfield Scientific, Inc. and its merchant processor to run an address verification search. This verification process is a security measure designed to protect me, the client, from illegal fraud against my credit card.
• Requests to terminate authorization of recurring billing must be made in writing 30 days prior to the next scheduled payment to that address listed at the bottom of this form.
• A $25 USD fee will be assessed if the card provided is declined for three consecutive days.
